Insurance Information
We are currently in-network with, and therefore able to directly bill, Aetna, and any Memorial Health Partners-based policy.
Although policies should cover lactation, we strongly recommend that you confirm this regarding your particular plan PRIOR to your appointment with us. Any portion of the visit not covered by your carrier will be directly billed to the stored card on your account. Remember, if you have NOT met your deductible (as per your insurance policy) you will then be responsible for paying for the visit.
If your baby is covered by a policy other than the one you are on, we will also need that information. If their policy is one that we are not in-network with then you will incur a charge for their portion of the visit, as our services are billed for both clients evaluated and treated.
We have partnered with The Lactation Network in an effort to increase access to coverage for lactation services.

If you have private health insurance (BCBS, United, Cigna, Humana particularly) please complete the requested information here. Lactation Network will evaluate your coverage and contact you with a determination. Once coverage has been established you may then book through our scheduling system via the Book Now button.

****All HOME or VIRTUAL visits, to be filed under insurance coverage, will be assessed a $75 convenience fee. This is not billable to your insurance and will be billed to the card on file. Please note, some insurance policies will no longer cover virtual visits. Confirm coverage PRIOR to booking.

YOU MUST add your insurance card and driver’s license to the client portal when scheduling your appointment to allow Latch to bill your insurance policy for your appointment.

Any other commercial insurance carrier
(not Medicaid or Tricare)
You may complete an application for insurance coverage here.
You should receive an email determination within 24 hours.
After you receive the determination then you may book your appointment.
If you do not receive a determination within 24 hours, please email care@tln.care to confirm that they have received your application.

Self Pay
If you prefer to pay for your appointment out-of-pocket, we will take payment at the time of booking. For any insurance, other than Medicaid, we can provide you with a Superbill that you can use to request a reimbursement from your insurance provider. We are also able to accept HSA/FSA as a form of payment.
